In order to obatain refund check , you must send in original UPC from KRAFT 2% Natural Cheddar Cheese along with the completed offer form (Offer form can be obtained by link provided). Mail them with the original,store-identified, cash register receipt dated between 3/15/09 and 9/30/09,with date and price of item listed above circled.
